Alternatively you can request a membership package by calling (416) 491-5932, Toll Free (800) 876-2372 or emailing us at: membership@cdpa.com.

Once you receive the application, please complete the document and include a valid email address and cell phone number. Your application for membership will be reviewed and you will be contacted by one of our Administrative Staff. It is imperative that you inform us of any incident or possible situation that has or could lead you to request assistance in the space provided on the form. It is in your best interest to do so.
